NHS Vouchers

Who qualifies for NHS Optical Vouchers?

You can get vouchers towards the cost of glasses, or contact lenses, if you:

  • Are under the age of 16

  • Are aged 16, 17 or 18 in full-time education

  • Need complex lenses

  • Are getting, or your partner gets: Income Support, Income-based Jobseeker's Allowance (Incapacity Benefit or Disability Living Allowance do not count as they are not income-related), Pension Credit Guarantee Credit

  • Are entitled to, or named on, a valid NHS tax credit exemption certificate

  • Are named on a valid HC2 certificate

(If you don't have an NHS Optical Voucher and think that you may be entitled to one, you can obtain a form HC1 from your local opticians, or your local DSS office. This form guides you through the process of applying for financial help towards spectacles, sight examination and dental work.)

What do i need from my optician?

You should ask your optician for your fully completed GOS3 Optical Voucher when you have sight test. Your optician is entitled to give you this providing you qualify for NHS help.

How long is my NHS Voucher valid for?

A GOS3 Voucher form is valid for two years, but you can only use it if you are still entitled to a voucher for glasses or contact lenses when you order them.

How much will the NHS pay towards my new glasses?

The actual value of your NHS Optical Voucher depends on the power of your glasses prescription. This is indicated by a code (A,B, E or F) that can be found on the front of your GOS3 Optical Voucher form.

Code        Value
A              £35.50
B              £54.00
E              £61.40
F              £78.10

Check the reverse of your NHS Voucher form. In some cases, a Full value voucher cannot be issued, and will be reduced by the amount shown on the back of the GOS3 form.
